Flexible Plans, Midwestern Service

Quartz competes head-to-head with national plans while rising above the rest with Midwestern friendliness and service excellence.
Offerings include health maintenance organization (HMO), point-of-service (POS) and preferred provider organization (PPO) plans, as well as narrow network options. We also provide administrative services only (ASO) and third-party administrator services (TPA) to self-funded groups.

Virtual First Program: $0 Cost-Share for Greater Access and Savings

At Quartz, we care about bringing your employees or clients access to health care when and where they need it. We also want to help everyone save on health care costs. This is why we’re especially pleased to bring you “Virtual First.”

Virtual First puts our members first, providing convenient access to primary care, urgent care, and behavioral health providers for $0 cost-share* when they utilize telehealth or virtual visits.

Visit with a primary care, urgent care, or behavioral health provider through the use of a telephone, smartphone, tablet, or computer.
They’ll enjoy the peace of mind getting the care to help them live their healthiest – when and where they need it – without traveling to their provider.

*$0 cost-share applies only to telehealth or virtual visits for primary care, urgent care, and behavioral health. View your Summary of Benefits and Coverage to see when cost-sharing for other telehealth or virtual visits may apply. Virtual First is not available on all plans.
