Travel benefit
This plan travels with you! For three months at a time, up to six months a year, get covered services at in-network costs when traveling outside of the states of Illinois, Iowa, Minnesota, and Wisconsin. Plus, members have access to most retail pharmacies nationwide.

Dental wellness
Get preventive & comprehensive dental coverage for services with a Delta Dental network dentist. Open network is also available with balance billing; $850-$1,550/year, depending on your plan.
Plus, add $1,000 of comprehensive dental coverage for $36/month.
Obesity and Type 2 diabetes reversal benefit
Eligible Quartz Medicare Advantage (HMO) and Dual Eligible members can join the obesity and Type 2 diabetes reversal program from Virta Health. This program can help members lower blood sugar, reduce the need for certain medications and achieve a healthier weight.
Virta is available at no extra cost* to Quartz Medicare Advantage and Dual Eligible members with Type 2 diabetes. For members interested in safe, sustainable weight loss, Virta is also covered for those with a BMI of 30 or greater.
*Additional costs may be incurred for labs obtained from an out-of-network provider based on contractual agreements between the lab and Virta Health.

Quartz Nourishing Meal Program
The Quartz Nourishing Meal Program benefit helps to support your food and nutrition needs. It is available to Quartz Medicare Advantage (HMO) and Dual Eligible members upon discharge from an inpatient or skilled nursing facility stay. It is also provided to members with congestive heart failure enrolled in a Quartz Care Management program.
The Nourishing Meal Program is a partnership with Foodsmart and offers:
- Home-delivered meals made specifically for your individual health needs
- Telenutrition visits with a registered dietitian
- A personalized food and nutrition plan
Blood glucose meters and test strips
OneTouch® blood glucose meters and test strips
Get access to high-quality blood glucose meters with Bluetooth capabilities that connect to the OneTouch Reveal mobile app, which automatically logs readings, tracks patterns, and enables members to share data with their care team. Our preferred brand of blood glucose monitoring supplies is the OneTouch® product line, which includes the OneTouch Verio Reflect® with backlit display, Blood Sugar Mentor™, One Touch Verio Flex®, OneTouch Ultra Plus Flex®, OneTouch Ultra 2® meters, and their associated test strips. All are available at no extra cost.
Massage therapy for chronic conditions
Acupuncture benefit
Gundersen Quartz Medicare Advantage members may receive up to 20 Medicare-covered treatments a year for chronic low back pain. You’ll need to see a licensed practitioner.
There is a $10-$20 copay per treatment, depending on the plan.
Hearing aid benefit
The hearing benefit Includes:
- One routine hearing exam per year with a $0-$10 copay.
- Get $1,000 – $1,500 every two years for hearing aids.
