Care Management

At Quartz, the goal of Care Management is to help members recover achieve better health in the right setting and in a cost-effective manner. Our Care Management services include:

• Care Coordination. A short-term, focused intervention that addresses potential gaps in meeting the interrelated medical, social, and behavioral health needs of members.
• Complex Case Management. Supports members with multiple chronic conditions. The RN Care Manager conducts a comprehensive assessment to develop a personalized care plan that focuses on helping members reach their optimal self-management skills.  This may include:
  • Education on diagnosis
  • Leveraging available community resources to address social determinants of health
  • Supporting members in building self-advocacy skills to navigate the health care system independently
  • Review of medication adherence to increase positive outcomes
  • Collaborating across Quartz, the broader health system, and the community to help members build optimal support systems
• Disease-specific Care Management. Programs focus on supporting members on their journey to a life well-lived with the following chronic conditions:
  • Congestive Heart Failure
  • Hypertension
  • Diabetes
  • Chronic Obstructive Pulmonary Disease
  • Coronary Artery Disease
• Healthy Beginnings. Provides support to our pregnant members. Our Care Coordination team is available to help connect members with needed resources to support physical and mental well-being. We can assist members in utilizing all the benefits their plan offers, such as doula services, our Virtual Village support group hosted by Quartz employees, obtaining a breast pump, and more.
• Healthy Futures. Provides ongoing support to our postpartum members and their growing family. Post-pregnancy looks different for everyone, and we are here to help navigate this journey by ensuring our members receive the care they need. Like Healthy Beginnings, Healthy Futures provides members with Care Coordination support to help navigate postpartum life.

All programs are available to Quartz members at no additional cost.
